For Non-English transcripts, a notarized English translation must also be scanned and included with your uploaded document. Be sure to keep a copy of the translation for your records.

Personal Statement:
The Letter of Intent is an important component of your application. It helps us to determine how well your interests match with our program offerings.In up to 500 words (2 pages, double-spaced) please indicate the following:1.Your aspirations, learning goals and career plans.2.What impact you hope to have on the dietetic profession.3.How the MPH Nutrition and Dietetics program will allow you to achieve this.
Recommendation Letter:
Two (2) letters of reference are required. Select referees who are familiar with your academic and career aspirations. Your reference should be able to comment on your academic ability and potential to succeed in a graduate program and as a Public Health professional or researcher. Generally, a former professor/supervisor or employer is the most appropriate reference. The University will contact your references using the email information you have provided in your application. You must enter an institutional email address for your references into the online application; a free email address (gmail, yahoo, hotmail) will not be accepted.
